Packer QB, Eddie Spankowitz. Back when steroids were first coming out, many players didn't understand cycle or proper use. Eddie over-injected into his neck, causing the gigantic noggin.

He became a mascot called Packy, IIRC.

I heard the Packers once had a mascot, but never saw a photo of a real person in it's role.
